Morrow Home Community is now hiring for a full-time MDS Coordinator! This individual will be scheduled for approximately 40 hours per week based on census. The MDS Coordinator will promote the physical and emotional well-being of Morrow Home residents utilizing the Resident Assessment Instrument to gather information about the resident and implement a comprehensive and person-centered plan of care.

Director of Nursing
Responsibilities
- Work with the Director of Nursing to ensure compliance with the requirements of the Centers for Medicare and Medicaid Services.
- Perform resident interviews and assessments necessary for the completion of the MDS Assessment.
- Process Medicare referrals and admissions, maintain schedule of the Prospective Payment System assessments as due and coordinate timely completion of assessments with the IDT.
- Complete all required OBRA MDS assessments including admission, quarterly, annual, and significant change of condition MDS assessments.
- Ensure compliance with the implementation of the initial and comprehensive resident plan of care.
- Review resident plans of care on a quarterly and as needed basis, update the plan of care as appropriate and ensure timely communication of changes with the resident, family and nursing staff.
- Coordinate the provision of skilled services for resident’s utilizing Medicare A or like-Medicare A benefits, which would include communicating with Medicare Advantage case managers.
- Review nursing documentation including nursing assessments, nursing progress notes, medication administration record, etc. as often as needed to stay current on resident conditions and service needs. Ensure adequate communication to Director of Nursing and IDT regarding this information.
- Attend in-services, conferences and seminars as directed by the DON to keep up-to-date on RAI processes and any factors affecting facility reimbursement.
- Assist with the coordination of resident care conferences.
Qualifications & Experience
- Registered Nurse currently licensed to practice in the state of Wisconsin
- At least one year of experience in MDS.
- Complete and pass a Knowledge Validation Test.
